It's just the way it's /so/ gigeresque, if somebody had shown it to me and said it was a work by him, I wouldn't have questioned it. Don't get me wrong, I love the style, but there doesnt seem to be anything about it that makes it /yours/, all I see when I look at it is a (admittedly VERY good) giger homage.

Hmmm I don't see pain here at all. The mouth is very relaxed as are the rest of the facial features but the mouth most noticeably. If you've ever seen someone in serious pain you'll understand what I mean. With agony there is tension in nearly every facial muscle, teeth gritted, lips pursed or drawn tight, brow furrowed, cheeks drawn in and downward. This creature's expression conveys to me a very strong sense of peace, relaxation, and pleasure. BTDT and I predict a cigarette followed by a long nap would have been in order shortly after the pic

For some reason the thing I really latch onto in this, visually, is her teeth. Thinking about it, I suspect it's probably because they are the most human part of her.
