I think she meant as in Tommy told her about Joel's birthday, not about his watch being a birthday gift from Sarah. If Ellie had known, I doubt she would have gotten him another watch, afraid to trigger any memory. We really don't know if Tommy knows about Joel's watch or not.

Because that it is the exact same dialogue from the end of the main campaign. Joel calmly lies to Ellie's face and she acknowledges that he is a liar and that she wasn't going to pursue it any further with that single word.
It's the realisation that, for Joel and Ellie, there would be no 'happily ever after'. They'll survive, maybe. However, this lie and the knowledge that Joel valued Ellie's life above a potential cure for the whole world will always lie between them; things could never be the same between them ever again.
