I always pictured the game being this way... After all, after the intro it shows him putting the intro pictures on his wall, and after the ending that show him put that picture up...

This has pretty much always been my perspective on the game.
I've heard so many theories about how Isaac kills his mother and feels remorse and blah blah blah...
... when the explanation that makes So Much More Sense is that it's all in his head. That it's a Child dealing with an abusive home by delving into his own imagination.

Well there are multiple endings you don't just get the rope. ._.

Either way, ending 10 to 12 and even the credits indicate that he did suicide.
10: A dead baby in a chest.
12: Isaac realizes that he is really corrupted and is ready to suicide.
11(Sheol Ending): Everyone that Isaac plays as (except corrupted Isaac and dead baby) openet the chest.
Credits: He sung a hymn while suffocating.

Actually, there's also a theory that ???, the final final final FINAL FINAL boss, represents Isaac's death wish. When you defeat him, you defeat Isaac's suicide wish, and ending 13 is Isaac letting go of his troubled past, ready to move on.
