Andrew, For thirdparty visual styles You have to use the uxtheme patcher or StyleXP installed.
uxtheme patcher found here :[link]
After install the uxtheme patcher than restart your computer & then copy the visual
styles file Black Aero Vista to your WINDOWS\Resources\Themes folder.
Enjoy.

The startmenu? well, since I saw the compact menu, I became fan of it.
Now, as for the startmenu vista button, it's not that I don't like it, but I see everyone doing it the same way all time, and I'd like something different.
