You need to use clipping, semi-transparent white highlights and blurred shadows. I would recommend you also to hit the "Download" button on this page and take a look at the source file in Inkscape.

You need to use simplified display modes (Ctrl+Num5 - toggle between modes). If you`re operating with big scale, Inkscape will work really slow, you`ll need to wait until the whole visible area will be rendered. But if you`ll zoom out, Inkscape will work much faster. So use minimum scale when it is possible. And you can draw some parts separately (on different layers, when you can toggle layer visibility to increase render speed) and combine them together later (make only on layer active, other layers should be blocked, select objects->Ctrl+X, select other layer-> Ctrl+Alt+V to paste exactly in the same place of the second layer.)

In some cases Inkscape could crash while trying to export your vector drawing into .png (It happened before with   one of my works). If it happens, the only way you can render an image is to pre-render some "heavy parts" before exporting: you should select objects->Alt+B (to make bitmap copy which will be located over the original objects) and then move these objects into another layer and hide them (toggle layer visibility).
