So far my favorite was "Super" Kaioken, when Goku used Kaioken during his Super Saiyan state in the Budokai.

Well by canon the frieza saga was the last time he really used Kaioken while in the outworld budokai was filler.
Its said that SSJ is ones x50 times so Kaioken wasnt used anymore since SSJ state can multiply same as Kaioken but without the self damage issue.

I dont think so,SSJ  is a transformation not just a power boost and look despite the idea of it,its simply a new pack with other kind of abilities and mixing it with Kaioken might just result into an instant explosion even activating the tehnique nevermind doing X3,4,10 etc.
Some people say Akira simply forgot about it,oh well we never will know.
