I had the same bug, it is an OS problem.
Last time I used pmd I had to check my locale on the PC, because everytime I saved it increased my Amai's model size. I had to change my locale and restart my model again... then it strangely worked.

there are several steps you will need to do before we can help you with your problem.
1 - PMD or PMX file format? PMX should have less problems than PMD format.
2 - If you are using PMD format, the maximum vertex limit is 65000 vertexes. Having more than this will cause the model to look bad. [i dont think you have this problem though]
3 - Run the PMD/PMXeditor error checker [File> PMD status (I) in the PMD editor, or File> PMD status (F) in PMXeditor)]. This will usually detect any problem with your model automatically, or at least tell you where the problem is. Stuff which are OK are labelled OK, and stuff which isnt usually will have an X. Let us know which part of your model had an X. But you should be smart enough to guess whats wrong with it.
4 - the usual problem is a bone order problem. Usually the crash happens because the connection is like: bone 1> bone 2 > bone 3 > bone 1, which will let your model be opened in PMXeditor, but will crash in MMD because of the endless loop.
5 - There is a maximum size for texture files per folder limit. Safe value is 15 MB. But if you go above it, you need to use PMX format and separate your textures to different subfolders, or else you will get an error that will never go away, but wont tell you what the error is.
If you want, you can send the model to me so that i can at least check whats causing the problem, but i may not have time to fix it for you.

a mi me paso una vez, y todo fue por un joint que no tenia fisicas puestas, y despues por otro joint que tenia una configuracion erronea... no se si eso te sirva :/

It could be a virus rooted in the file that increases its size. I have heard of viruses that do that. :C
