It's a little joke from the last chapter of the Naruto manga. A lot of people were happy Sakura finally 'Blossomed' and I thought of an episode of 'Itazurna na Kiss' where the main character Kotoko was pregnant. Both girls were nearly flat chested during their stories and when Kotoko was pregnant her breast finally grew... the scene with her looking in the mirror is hysterical. So I thought Sakura would probably do the same thing when she was pregnant with Sarada.
