That was done using Zbrush, 3ds max and hair farm to create the hair.

Hair farm is a max plugin. You can check it at [link] if you are interested. It doesn't take too long if you render the hair in scanline and compose it later.

They are modeled the same way as I would do with a realistic character, they have the transparent cornea and the eyeball inside.
