It's cyanogenmod's default launcher, I think it does. Not sure how it works though.

Sure it does, install the icon pack, hold down onto the homescreen and you'll see the 'Icon Pack' option on the top right corner.
PS. This option will not come up if you don't have any icon pack installed.
