Ah, I just love monitors... On a Dillinger Escape Plan gig one time, I got hit by a monitor that was kicked by the bassist 
And don't be surprised of the DD, you definitely had it coming!

So far nothing serious happened to me in the photo pit. But last year, some guy in the audience pulled my hair during Overkill gig just because I got really close to the barriers. Of course, almost instantly he got what he deserved.
And yes, shooting on the tip of my toes got me a DD
