I got mine from Wal-mart. The dark t-shirt ones are the best and most durable. The white ones crack and don't hold up well on anything. They're about $10 a pack. You can find them on ebay as well.

um...basically you print pictures out on them and iron them on to 100% cotton stuff. T-shirts, fabric, whatever you like. They're really easy to use.
