Do you start off with a sketch or just by exploring and forming shapes with a large brush?

I don't really have one fixed way of doing things. If I got a clear idea or have to fit in a certain criterias (design wise, composition wise, etc) then I may start with a rough line sketch or 3D base, just to be sure that I got everything in the right places from the beginning.
If I'm free to explore, then I may start off with wide brush strokes and then maybe get some unexpected results, good or bad. I also can start off with throwing in different photos, or with messing around in 3D, or use custom shapes and brushes. Whatever works, really. As long as you respect your fundamentals, you'll be able to figure out the way to make things work.
