this reminds me so much of Harry Potter and the chamber of secrets, the rats(including this monstrosity that seemingly looks like a giant one) are like both the actual chamber with the basilisk and the spiders.

I have seen it before, a demon have possessed that big rat. Possession usually twist the body in strange and macabre ways. My best guess is that it is a greed demon, and considering that it only took a rat, it is one of the lesser of its kind, if a demon had managed to posses a dragon then Erma would have been in trouble.
