In the roleplay, Sasuke is blind and is having mixed feelings about living. He's captured by Leaf ANBU and is dumped in a cell until Naruto convinces Tsunade to let Sasuke be taken under Naruto's care and guard. The 'X' on his chest and the blood is a mental wound from being conflicted about life and Naruto. On one side, he wants to give everything up since he lost his eyesight and can't exact his revenge, but on the other, Naruto's presence is preventing him from taking his own life.
