Nicely done. And I do believe Illidan's commitment to being shirtless all the time is so strong, that not even being a girl would stop it. I site him not wearing a shirt in northrend as proof.
As for him being a girl in the story, well I'm not so sure about that. The love triangle between Illidan, Tyrande, and Malfurion is a pretty important part of the story line. If you made him a girl then the story would certainly gain some popularity in some circles, but it would lose some of its mass appeal and "relatebilty". You could try reversing all three, but I can't see Malfurion as a women. Illidans most recognizable features are his blindfold, his tattoos, his Glaives, and later on his wings and horns. While Malfurions are his antlers (which wouldn't be a problem), his awesome lumberjack beard, and treetrunk thick arms and torso (Both of which would be a problems).
One more thing, why is the skull so small, did you turn Gul'dan into a girl too! Because I don't even want to think about that. (But seriously Great job in the pic, love the transparency in the wings.)
