Description
A dark side of Sasa.Since over 20 years he is on the needle and takes hard drugs. Luckily it won't kill him nor damage his body, as vampire he keeps the shape how he was when he was turned. He doesn't age nor can drugs or poison in an kind kill him, even if they have the about the same effect on his body like on humans. What is ways worse is the psychological dependency. He went through some withdrawals already, but he could never keep his fingers off his stuff for longer time. Every time his life got more troublesome or painful - and that happened quite often - he escaped.
Even today he still takes drugs. Marius doesn't know about, the only vampire who knows is Kenji as he was the one guiding him through withdrawals in the past - not on a professional way obviously, but he tried to help him at his best. Sasa fears heavily that his addiction one day will destroy his relationship to Marius. And pressed by this fear he again escapes to his drugs. An evil circle barely somebody knows about.

A quiet ear to listen, a safety net of a person if you will, is what he needs. I agree that the psychological dependency would be worse. The physical symptoms have something that can easily be dealt with, signs that show obvious improvement or the like. The psych stuff...that's a lot harder and a whole other mess.
As for the pic itself, I like the shading and way that you portrayed him. Right now, he looks very vulnerable. I like that you did not put him in his normal clothes. If he'd let them, I know a quartet of Elves who would keep him, and his secret, safe.

The physical symptoms are heavy as well, and Sasa fears the massive pain and the shame when he is about losing control over his doings, but he also knows that it will ease within several days/weeks.
More problem is for him that he is simply unable to speak about his addiction at all. And he would never start talking about it by free will. Kenji knows about it simply because he had seen him shooting up and he knew that he sold himself for drugs in his past. But Marius doesn't, nor the rest of the bunch (some do, but they don't know that Sasa is still addict), simply because Sasa not speaks about it even if his life would depend on it. By that he also is also can't really make up a safety net for himself.
Kenji was officially his enemy in the past, belonging to the Clan he was fighting against and by that also not available all time. The withdrawals Kenji tried to help Sasa with also were quite rough ones, often cold withdrawals without medical support and he had neither the knowledge about what to do after the physical withdrawal nor the possibilities to keep an eye on Sasa and watch over his condition.
What is about nowadays after the Clans broke apart and Marius took over the leadership I won't tell too much yet, as it would spoil the happenings of my story. But so far I can tell, Sasa will hit his nose by his silence - at least they do have a Doc today xD

I see. Those are two perfectly valid things to fear, when one gets down to it. And the way that he can look at the time they will fade makes them slightly easier to deal with.
That is fairly typical and I'm glad to see that you aren't ignoring that. I know it is embarrassing for Sasa but it is good to see that you have kept him 'real'. True enough--no outreach, no safety net. I am happy to hear that he has someone to help him through the hardest parts.

Without Kenji Sasa would be in really serious troubles. He owes him a lot and Kenji was the one who picked him up quite often to take care of Sasa, even if he risked his head by it. To help an enemy was something what was punished bad. Often Kenji took Sasa as captive and let him escape after a while again to make up a reason for Sasa to stay in Kenji's house. It was a hard time for both of them, but their friendship is very deep and close and nothing actually could break it. Next to Marius Kenji is the most important person in Sasa's life.
