I think that someone on tumblr pointed it out. So I think that what happened was, Steven was a fallen child, and he died at some point and became a skeleton. He changed his name to Sans, and met Papyrus. They shook hands with spit (or whatever the skeleton equivalent is) and that made them "brothers."
