I've read a LOT of theories about that eyeball.

That it's an implant, an experimental thing inserted into his skull that hyper-focuses his magic. He volunteered for the experiment, and it nearly killed him.

That all skellies glow their eyes when feeling a powerful emotion, and Sans's right eye is sort of broken.

That while his left lights up, his right can see beyond where he is when dark, sort of like teleporting his vision somewhere else. That's how he knows exactly where the player is gonna be at any given time.

So many theories... those are the ones that come to mind, though.
