You have to unlock a friend area ("Piana Cieloterso" in Italian... It's the friend area where Smeargle is so... I guess You've already got it...)
Ekans and Medicham will talk about the Wish cave for a few days (you'll find them next to the place where Pkmns evolve)
You have to save Medicham from the Wish cave before (difficult dungeon, but you have just to reach the 20 floor)...
And then he will ask you for help OwO and the episode starts...
