Question: How do you keep people from using your designs without paying? All they would need to do is redraw it on a character, or change it slightly so it’s not exactly the same and you would never know?

Word travels fast on DA. The community is pretty good about spotting someone using another user's design and bringing it to their attention. I found one by accident, of a design I had custom-created. I was looking at a piece of art and thought, "...this looks really familiar." I finally caved, reached out to the artist, who had no idea the person who commissioned the piece had used a design of mine without asking and immediately took it down.
It used to be when you commissioned a piece from an artist, you had to verify the original character design was yours, or that you got permission to use an outfit from either a user or an artist via screenshot. I think it got outdated quickly because people couldn't produce the right evidence or didn't know how and artists were losing out on potential sales.
It's also really a show of faith. We hope you'd at least ask before trying to make off with it.
