It's a different scenario where Frisk returns to the surface, only to be taken to an asylum because her talk of monsters made them think she might have hurt her head and cause brain damage. Because she doesn't return, the underground lose hope and become more violent. Until Sans was able to get out...how? I'm not sure. I think he was chasing Flowey who lead him to where Frisk was. How did Flowey know where Frisk was...I'm not sure on that one either.
