Very realistic! He looks like a guy I might see on the street (with a shirt and shoes), but I can still magna/anime Itachi's features in him. I'm especially impressed with how natural the veins, skin texture, and overall anatomy look. I mean, I'm not anywhere near an expert or anything, but everything- the veins, the arteries, the shadows, the bones, the muscles, the tendons- look like they're where they're supposed to be. Your depiction of his hair- the hair on his head, stubble, and, uh, lower- is also very nicely done, very realistic looking. This is an Itachi I can actually imagine as human. Also, hella cute.
