Well, the title. as you know, is "Looking back" and her heart is on her shoulder/back as she ooks back at it. Kind of like, that love, or that part of her is now gone. It is no more in her,yet, it's still in site and she's looking back at it, remembering how she felt when she loved. Etc...
I always try and pull symbolism out in things, even if it wasn't intended by the artist >.<
