How exactly do you go about designing such a fine weapon? Digital? Sketch first? Very curious.

1st I make a sketch.
2nd I line it on new layer.
3rd I apply flat colour on another several layers.
4th I add shadows 
5th just add some more finishing touches if I feel a need to.
