In the TCG and the like, a lot of the time, it's insinuated that affliction warlock's practice with sicknesses and magical diseases and whatnot show up on them, like a virus showing up on them to make them immune to their own stuff.
And I think it's why it's more rarely practiced than the other two specs, afflictions show on them like horns /claws/demonic stuff show up on demo 'locks and destro's get on fire when they're angry/intense emotions and all.
But, I am pretty sure it could be 'hidden' at least with controlled emotions and whatnot to a point. When my Dark Iron lock's in a city, he's got a few boil-like bumps and what looks like an old burn on the side of his face.

To be honest, I'd never roleplayed an affliction warlock until the patch. I didn't know all these things, but it makes perfect sense! Thank you so much for explaining this to me!

It's all good! The neat thing about affliction locks is that they're so very rarely played (at least , non-Forsaken affliction based 'locks), which makes you original in a sense.
You can also make up all sorts of diseases and sicknesses to use at your disposal.
