It's unknown since mina has never told anyone about it,including her brother and the queen.
She keeps it a secret. But who knows,probably an accident or an enemy. But she did get that scar when she was 12.

Surprisingly sasha aka cailys daughter was born with a symbol instead of being trained.
The others such as aaron and gandy were trained and got their symbols.
