She's a background character that had an animation error in episode 1, and ever since then, she usually appears with her "derped" up eyes. She appears here [link] at 11:09, here [link] at 13:14, here [link] at 17:33, and here [link] at 0:37.

It's not her real name, it's just a fan name. I'm pretty sure her "real" name is Ditzy Doo. She doesn't have an official name though
