It's very strong, saying you're ok and just go on, but it takes way more strength to show a weak spot. It can bring you more though.

The people who ask usually don't care and just ask out of courtesy, so why bother telling them...
Friends... if there is actually someone you can truly call a friend you know they will eventually tell the school councilor and/or your parent about it because they do care...
Family... sometimes part of why you are not okay... or you just don't want to burden them any more.
Saying ''I'm okay'' and faking a smile just seems easier, well to me anyway...
