She has a goregously sad expression. Her shoulder joint is protruding at an angle that looks painful - if she weren`t a doll. This adds to an overall feeling of exquisite melancholy that holds her down. A wonderful photograph.

This is the most beautiful doll face I've ever seen.
The pose, along with the expression, is evocative.
The body has a realistic look--either a good materials choice for quality, or good workmanship, depending on your techniques. There is also good, subtle lighting on it.
It's all very well lit and photographed in B&W.
I'd conclude there is extensive artistic talent involved.
