You must check your bone chains. Each bone has a "parent" bone whose motions it follows when that one moves. Hopping from one parent to another, you should be able to trace each bone's origins up to the center bone (except leg_IK bones that are connected to the motherbone directly) and from it to the motherbone (whose parent field contains "-1", which means there's no authority above that). If your model does not contain a motherbone (for example, stock models from MMD do not, for some reason), then the center bone *and* leg_IK bones have "-1" as parent, though it's a good practice to add one (Menu: "Edit->Bone->Add Parent", the last command in the list). If there are bones whose ancestry can't be traced to one of the above, then that's where you messed up.
Normally, your model should contain no less than four IK bones: two for heels and two for toes (though some can also contain IK bones for braids, ties, hands, etc, but let's not talk about complex stuff for now. Now, in your screenshot I can see no less than ten free-moving bones at the foot level; well, one of them might be the motherbone. but what others are for?

Thank you!! I actually restarted the model because I did delete an important bone by mistake!
I appreciate you guiding me to this tutorial, I'll definitely use it when I run into this problem again.
