I use the vectors tools in Adobe Illustrator and trace paths. Photoshop offers a few path tools, but Illustrator is designed around em.

Well photoshop gives you access to the Pen tool. This can be used to draw more complex shapes than the default squares and circles.

I'm not sure if you can combine shapes like you can in AI, but I bet if you get creative with the layering and masking it could work.
