It's a good start, though I do find it hard to tell whether Sonic's gender was originally female as I was trying to imply, or whether Sonic had been transformed into a girl...

No, she's female originally. She didn't know it. Hedgehog parts are inside of them in reality, my father told me that once. And remember, there are no mention of parents in the series so I'm assuming in this headcannon they left her young or were killed so she had no one to teach her the ways of life, if you like. And since she had quite a deep voice she just assumed she was male, also considering the fact she'd now be considered a tomboy. Who else would have told her? Boys don't tend to talk about that sort of thing together. They wouldn't have seen anything. And she hasn't developed breasts this early on in the story as she's a late developer at 15. Soon enough that'll be her next big problem. Apparently periods can be delayed if you do a lot of sport and Sonic runs a lot.
It's a headcannon that could actually work.
But now he knows, I think he'll be keeping it secret for a while. Something as big as that...well for starters Amy was in love with a woman all this time, that'll be hard to tell her |D
The doctor will explain it all next chapter and there may be some awkward moments. I'm just looking forward to how he will tell the others.
I gave it my best shot at about 1am in the morning. I think you should have a go and get some friends to have a go. It's interesting to be given the same plot and see how many different interpretations you can get. Sometimes the stories will differ greatly.
