Description
There's an enemy called Shyren that you can hum with to start a concert. If Papyrus is alive, then Sans will show up to sell tickets to the concert. If Papyrus is dead, instead a hooded figure will be watching you. It's possible the figure isn't Sans but I think it's really unlikely.Which means that if you spare Shyren at that point, Sans will be watching. Can you imagine how that would make him feel?

shyren is a scripted encounter, aka they appear once in the same area every game.
if you kill papyrus and continually "hum" with shyren, you should get the hooded figure.
i havent tried this, i will after i finish my genocide route, but thats what i assume.

I think, maybe, after you kill Papyrus, since Sans doesn't show up visually again until the castle corridor, and since he's watching you at Shyren's concert, that Sans is actually following you around while hiding.
Like, he probably saw you kill Papyrus, and even if he didn't, he still knows you killed him. It would make sense for him to follow you around through Waterfall, Hotland and the Core, until you reach the corridor, watching what you're doing. He doesn't care if you kill any other monster, but kill Papyrus and he'll be stalking you for the rest of the game. Really creepy thought, but I guess it's deserved...
I've been playing so many neutral runs these last couple of days, killing certain characters just to see who ranks higher to be elected new leader, until no leaders are left. Turns out you don't need to kill everyone for the Underground to fall into despair, just those who will step up as leaders, which are actually really few.
I had to kill Papyrus twice for this...
Now I'm doing the one where I start genocide but abandon at Papyrus. I need him to set me straight again. ;_;
