As a demon, Dean has picked up a lot of bad habits, including smoking.

He finds that blowing out parts of his own blackened soul along with the smoke is an interesting experience. So he allows part of his soul to leave him, being both in and out of his body; it’s almost like a drug high.

His tattoo is also warping from the power of the entity inside of the cage of flesh it lies embedded in. Burning and rotting away, leaving behind raw flesh and bruises.
