I got it done...can't believe it...my hands...my poor hands...So here is my vision of what could be the very first set of divisions in the Gotei 13, as described by Yhwach just before he finished off Yama-jii. Watching over them, always watching is the Spirit King...rumours say he could even appear himself in the doujin soon...we shall see....
1st Division - Yamamoto Shigekuni: A demon of the sword. After defeating King Yhwach and establishing the first divisions, Yamamoto founded the Gotei properly, alongside the Shino Academy.
2nd Division - Asashin Shihoin (OC): The first leader of the Onmitsukido. The legends say that no one ever saw her face behind her veil, not even the Commander.
3rd Division - Nimaiya Ouetsu: An exceptional blacksmith who forged the Asauchi. Ouetsu was later promoted to the Zero Division.
4th Division - Tenjirou Kirinji: An exceptional healer who forged the healing hot springs. After passing his knowledge onto Yachiru Unohana, Kirinji was later promoted to the Zero Division.
5th Division - Ichibei Hyosube: The second Shinigami to made a captain after the Commander himself. With an absolute knowledge of reiatsu, Ichibei was the first to be promoted to the Zero Division.
6th Division - Tsujishiro Kuroemon III: A master noble weaver who would later go on to craft the famous scarves of the Kuchiki house.
7th Division - Kemono Komamura: The first of the secret Komamura clan to make contact with Shinigami and learn their ways. After the war with the Quincy, Kemono retreated into hiding and regressed back to an animal state. Kemono is the grand-father of Sajin Komamura and is currently doing battle with him.
8th Division - Kyoraku Shunsui: Still extremely young, Kyoraku was one of two to be promoted to captain first from Yamamoto's new academy. He would later go on to become the Captain Commander.
9th Division - Kyōfu Hisagi (OC): The great-grandfather of Shuhei Hisagi and founder of the Seireitei Bulletin.
10th Division - Kukaku Shiba: Still extremely young, Kukaku was a valued Kido expert who would later go on to join the Zero Division, her right arm being used in the Ōken creation process. She was removed from the division after the Shiba House fell.
11th Division - Yachiru Unohana: A diabolical criminal who stalked the Rukongai, Unohana was taken in by Yamamoto for her skills. She is the progenitor of the Kenpachi name and only officially passed it onto Zaraki Kenpachi.
12th Division - Senjumaru Shutara: A brilliant mind, Shutara was very young when she joined the Gotei. She taught a child Mayuri as her protege, before being promoted to the Zero Division for her creation of the Sutra Scale, which helped maintain the balance between worlds. Coincidentally, her successor, Kirio Hikifune, would also later be promoted to the Zero Division.
13th Division - Jushiro Ukitake: Still extremely young, Jushiro was one of two to be promoted to captain first from Yamamoto's new academy.
Of course, all of the above is speculation pretty much. If you like, you can consider it the official lore in my doujin

Kyoraku and Ukitake were just kids around this time - if they existed at all. Back in the chapter where Yamamoto dies there is a flashback to when he explained Yhwach's story to young Shunsui Kyoraku - and he was a kid. Also, I think Kuukaku is too young as well to have been a part of the first divisions. Maybe one of their grandparents would have made better choices as captains of the 8th, 10th and 13th divisions. Other than those I think this is pretty good.

Kukaku shiba is certanly too young to already exist in that time

The only alterations I would make is, with no OCs,...
2nd division- Senjumaru Shutara (1000 years ago was likely before 2nd squad and the Secret Mobile Corps were merged, and even then but we know the Shihoun clan heads were traditionally leaders of the Secret Mobile Corps) A Shihoun OC, Yoruichi's grandfather or grandmother, was likely the current head of the Secret Mobile Corps. And even so, Senjumaru is, afterall, the secret keeper and taker.
6th division- A young Ginrei Kuchiki. The reason for this is because I feel they completely forgot about poor Ginrei when listing off the existing members from 100 years ago or more, especially since the only reason they remembered Unohana is because they were afraid of her. Also I can't imagine Ginrei being younger then Shuunsui or even Sasakibe. (We know too little about Tsujishiro Kuroemon III to know whether he was a captain or merely an artisan)
8th division- Mayuri Kurotsuchi. While being a vice-captain during a hundred years ago, as well as being one of the weaker captains kind of makes use hard to imagine, the fact is he confronted Yamamoto about how he didn't kill Bach completely 1000 years ago, knowledge only a fellow captain would know. (As others have pointed out, Shuunsui was still a kid)
9th division- Ganryū Ryōdoji. Unless Kubo is saving the reveal of the 4th house as Komamura's clan (unlikely), the Ryodoji clan from the 1st movie is the 4th head clan, but they were all exciled for their brutality. Ganryu matches the description perfectly (As other's pointed out, Hisagi was from the Rukongai, so a relative makes no sense.)
10th division - Bansui Amatsuki. From what little we know about this person, he was one of the greatest genius' of soul society as well as Kisuke Urahara's mentor, as well as had immense spiritual pressure (As others have explained, Kukaku doesn't work as it cause she is stated to be only Urahara and Yoruichi's age. I expect she was 3rd seat of the kido squad)
12 division - Kirio Hikifune. Already stated division head of 12th squad
13th division - Isshin Shiba. While it is true that he looked really young just 20 years ago, we have to consider this as a possibility. 1) He constantly talks about Shinigami lore, as if despite being kind of dim by his own admission, he knows things that have been done by shinigami for thousands of years. 2)The fact is that he is a lot stronger with his release-less zanpakutou, finger flipping Aizen and releasing giant energy Getsuga Tenshou's, then he was back when he had a bankai that merely drained his stamina...suggests a power he did not expect(since Aizen could of defeated Urahara and Yoruichi, but not all three without the Hogyoku luckily activating), and 3) He is only other user of the Final Getsuga Tenshou, meaning he has lost his powers another time in the past. It also means that Isshin holds a power unknown to even Urahara or Aizen. There was a point in time where Isshin's real abilities were a complete unknown. Its also possible he was de-aged by its usage, and Kaien Shiba joined the 13th squad cause the Shiba clan had ties to that squad.(As others have pointed out, Ukitake was the same age as Shuunsui)

The idea of Komamura's great grandpappy being a member of the Gotei is a decent idea. However, I'm pretty sure Ichibei is going to be revealed as the creator of Kidou and the founder of the Kidou Corps. The idea of Tsujishiro being the captain of 6th is also really clever.

Hands down, one of your best work (Even though I wouldn't say its a 100% accurate) but I think you are right of Kukaku,because if you read "The Dagger 4" it will mention if Kukaku and the Royal Key are connected.

I think Nimaiya was the first to be promoted. (strangely can't see soul reapers without their zanpaktou. I even think he's older than old man Yama, despite his looks)
Shunsui was shown in a flash back, asking what that poster of zanka no tachi was, showing he was still a child. (I think he and Jushiro were to young than to become captains)
as for Kukaku, she was the younger sister of Kaien, who much later became a lieutenant
I agree with Shutara, except for her protégée. I think she's the one who locked mayuri up in the maggots nest, which is why she was surprised to see him. (and mayuri angry to see her)
