He isn't really related to Scrooge at all. At least not by blood.
Gladstone's mother Daphne was the sister of Donald's father Quackmore, who married Scrooge's younger sister Hortense.
So Scrooge calling Gladstone his nephew is more of a pleasantry than anything else.
