Allow me to present Kaoru's twin brother. His real name is Misao as Kaoru reveals later in the story but he goes by the name of "Niou" most of the time due to a joke other characters played at his expense, by refering to the twins as "Kaoru and Niou", like the blood-related characters in the Tale of Genji, who nurture a vivid rivalry between them. While in the epic tale Kaoru is considered the anti-hero, in this story it's pretty much the opposite though.
Also, although writen and pronounced differently, their names mean exactly the same.

Niou was the first of the two to be born and ended up being drowned at birth by the okamisan who ran the okiya where their mother worked. Kaoru was more lucky as the other geisha managed to hide him away while Niou was being killed.
So, Niou lives in the spirit world now and his grudge against his brother has prompted his tail to start splitting. He is quite happy about that, actually, thinking that the further it splits, the more powerful he will become so he can once again enter the world of the living and kill Kaoru with his own hands.
However, he forgets that if he truly becomes an ayakashi (evil spirit), Tatari Mokke the yokai, will come to drag him down to the abyss forever.

Somehow along the story, Kaoru becomes aware of all this and realizes his propensity for accidents wasn't mere bad luck, and makes it his business to save his brother from his hellish fate.
