Headcanon time: Rick was very paranoid as a kid - not in the same way as Tweek, but more in a 'there's conspiracies I need to expose' kind of obsessive way. He didn't have many friends. Eventually his parents sent him to a mental hospital where he was diagnosed and put on heavy medication that made him sedate.
