Icons are a combination of Nanna (paid theme, but 100% worth it) and Cordyceps. For the dock get ClassicDock on Cydia (paid as well afaik).
No idea on the ls.

This screenshot is on Android (MIUI ROM), not iOS. I already figured it was a mix of those icons but I can't find this version of Cordyceps to download
