This species are both active predators and a sentient race? It's a little confusing but highly original. Do they have multiple cultures with some prefering a more animalistic existence and such?

It's a little more complicated than that, basically the medusaurs that remain on their home planet have basic tribal-like cultures with more animalistic tendencies, whilst a fair few of this species were initially captured by humans and taken off the planet in a sort of "illegal pet trade" for use as intelligent guard animals/fighting arenas. However, I the ones who were taken off-planet were intelligent enough to eventually adapt to this change in environment and learn human languages, or thereabouts, and were eventually classified as a sentient race and their use as slaves banned. The ones that had integrated into humanoid society then couldn't/didn't want to be returned to there home planet.
Maybe I should make the on-planet and off-planet Medusaurs separate reference sheets, they're not exactly different species but they do indeed have rather different traits and personalities.
Hope you don't mind the large paragraph, basically this difference can be explained as I've almost got two projects going on: A more speculative-biology based planet project where the wild-type medusaurs fit in best, and a sci-fi/fantasy universe with a plot that calls for more human interaction!

there's still one thing I don't get, the quadrupedal vs bipedal thing. Is it just optional, or are those supposed to be their wild ancestors or..

Well, they can do both. I imagine the wild ones do walk on more fours more often as they can run faster for hunting, unless using tools and such.
The "modern" ones are actually slightly different to their wild counterparts, I imagine they've evolved to be more intelligent, and they choose to walk on their back legs most of the time to free up their hands constantly. Also, they often bind their back legs into their knotted forms, and they fuse together giving them more strength to walk around bipedally the whole time.
The first of the "modern" medusaurs were probably taken from the planet, most likely illegally, to be used for things such as "guard dogs" and for use in arenas as a wild animal to fight. They probably didn't know medusaurs were able to learn human language at that time, but a few generations down the line and they proved to be more intelligent than they looked, and more interest was taken in educating the species for use as guards and things like that.
Eventually they were declared an intelligent species and granted all the legal protection they deserve, so most of the off-planet Medusaurs were descendent from the originally "smuggled" medusaurs. This is also the reason why there's a greater prevalence of unusually coloured and calmer medusaurs off-planet, as at the beginning they were selectively bred for these traits before being declared a sentient species. They also are able to understand humans better than their wild counterparts for the same reason (Something like how dogs were selected for their ability to communicate with humans). The increased use of technology in recent years has also selected for more intelligent medusaurs.
