Yeah they have cars, but Ash and May (or Misty, or Dawn, etc.) don't exactly have the funds to hire one. In-universe, intercity bus and rail service exist, but part of the whole point of a Pokemon journey is to go walk around where wild Pokemon are so that you can battle and capture them. They probably could get bus tickets to the next town if they wanted to.

I know Ash didn't just say "bike". Weren't the first, like, three girls following him around doing so because he (or rather, Pikachu) fucked up their bikes? I know that's what happened with Misty, and I think May, too.

I really liked the Pokemon setting through - small towns/villages mostly (with the occasional city) and lots of nature, yet everything seemed close enough together and harmless enough to get everywhere on foot.

to be fair though,every regions is pretty tiny when it comes to distance,they're more like large islands than countries

These kids do a lot crap. I mean besides walking between towns, they go camping out in forest sometimes, fight random killer animals all points of the day, kids pokemon world by our logic would have bitten dust becoming trainers or become like the world next survival masters.

There are also faster ways to travel in pokémon, you know.
The move "Fly" being the most well-known. But you still have to get to the place a first time for it to work.
