I don't see this as an anti-smoking ad but rather an anti-McDonald's ad. I read it as McDonald's is habit-forming (have you seen the US??) and causes mortality(obesity) like cigarettes. I see it as "take a bite of death", eat McDonald's and you'll be addicted.
..and can you really HATE smokers?
Its a personal choice..you don't have to like it but you're not the one smoking the cigarette now are you? It says more about your character and judgement by saying that statement than smoking a cigarette say's about a smoker's character. I'm just trying to say its one more form of prejudice we don't need.
