I believe you die if you get to this room and then leave without dealing with both the Warlock Lord and the Behemoth when you first enter the room.

I wouldn't be surprised if that happen. Hadn't played this awhile with the NES version being the only one I played. I only know using the staff on the Warlock Lord was one of the ways to die here.
