The only thing that seems out of place for a cenobite to me is the siamese twin since that doesn't seem like something a cenobite could or would do to modify their bodies

I like the baby hand. I think that if there were old man hand on top then young hands then baby hands that would make him extra messed up

Oh man this guy looks amazing! I could just see a story with him!. I see him as a WW2 doctor who (much like Mengele) specialized in research of twins or even a doctor who separates Siamese twins and he loved that he was doing, he loved causing pain to others and loved seeing blood. Or he could be an abortionist that took the lives of hundreds of unborn children and he liked what he was doing. But like most of the cenobites, they want more so they seek the lament configuration, open it and the Leviathan chooses them.

You definitely succeeded . I think this piece is to be viewed in less of a Practical sense and more a Shock/creep factor, much like the Hellraiser franchise

Very interesting take on a cenobite design. This guy could definitely be in a Hellraiser film. They are rebooting Hellraiser from what I hear, maybe you should see if you could pitch your idea for your cenobite to them.
