The disclosure of a medical condition is entirely up to the person whose health it is. You are right that the unnamed woman is a hypocrite. I think that's the point. I have a medical condition that results in packing on the some pounds (or kilograms), but I do the best I can to keep it in check. I feel like the point Adele was making was that one can take control of ones life. And having a high BMI is unhealthy. The unnamed woman is, IMHO, is shaming those people who have a low BMI because they worked for it. There is a saying, "there are none so blind as those that will not see." By saying that one can't change, ones creates a self fulfilling prophecy.
I would say, in contrast, there are people who have an extremely hard time losing weight. Personally, it's never made any difference to me what a person's BMI is. It's WHO they are on the inside that counts.
