Fairly good, though I think it could use a little polishing. As far as content is concerned, it's quite original; setting it up like a form letter gives it a good twist, but it looks too informal in its current state to me. I know it's a bit odd of a point to make, but I think that, if you used square brackets ( the '[' and ']') it would look more like a real form letter than by using the curved parentheses. Also, "instert name" sounds a bit too much of a cliche and seems to detract from all that's good about it--you might want to replace that with a blank, underlined space. On the whole, though, it still stands as a good bit of work.
