The second flag on the top left is, if im not mistaken, Turkish flag, yet in medieval times there was Ottoman Empire, which had a different flag; a green banner with 3 crescents

They had many flags, as there was nothing like a "national" flag; red and green, one crescent, or three, the military flags showed the sword of Jakob (I think, I mean this double bladed item) etc

I don't think the Vatican, Russia, and third banner from the left in the last row were used in medieval times. Also, the lilies in France's banner should be 2:1 instead of 1:2 and I have never seen that Mameluck ensign outside of EU3. Top row second from the right also is weird.

Third banner from the left in the last row is the *alleged* flag of the Kalmar Union. Some claim it was used, though I personally disagree. Top row second from the right is the Hussite symbol used in Paradox games. While the chalice was (and is) a widely used symbol in Hussite theology, I sincerely doubt they had a flag quite like that.
Damn Paradox and their misleading, incorrect and anachronistic use of flags and symbols sometimes...XD
