The described kind of person, has often a hard core inside that soft personality that everyone can see. That core is made by years of pain and suffering in a hellish environment. Not all people make it, out of there, in time. Sometimes it's a monster armed with sharp teeth and blades all over it's body, indestructible and covered in what only looks like that very sweet person that was very strong inside, but is only skin and bones now. And often, it doesn't even know it yet, it harms others on it's search for warmth. On realization it may suffer even more or like the taste of others suffering. This may not always have a happy end.

Except that the ones with the plan, only have a plan that they came up, to satisfy their impulses, but preset the kind of victim they want and what they do with it. And the ones who act on impulses, made the procedure a emotionless and cold, trained routine, that they don't allow any errors to be in. Their impulse in it is, that they react with a reflex to a suitable victim. But you are right, it's a bad example for this case. Looking back on your other comment, I actually understand why you see insanity as counterpart to rationality. It seems you think that social laws, the incorporation of consequences and emotional control, are important parts of the rational though process. I probably am a better example, now to describe me, think of a person that doesn't care about the existence of social laws, has a mix of philosophy and math as indicator for worth, also first thinks about how to execute a task at hand and only second, if at all, about the consequences.
